Agreement Forms

  • Troop Product Manager Agreement Form: Fill out this form to opt-in to your role if you are serving as the Troop Product Manager for the 2019 Cookie Program (and did not fill out this agreement for the 2018 Fall Product Program).

  • Service Unit Product Chair Agreement Form: Fill out this form to opt-in to your role if you are serving as the Service Unit Product Chair for the 2019 Cookie Program (and did not fill out this agreement for the 2018 Fall Product Program).

  • Juliette Parent/Guardian Agreement Form: If your Girl Scout is not participating in the Product Programs with a troop, please fill out this agreement form 2019 Cookie Program (if you did not fill out this agreement for the 2018 Fall Product Program).

Cookie Materials (Troops & Girls)

  • Cookie Dough and Reward FAQ: This document covers updates to the Cookie Program’s Bonus Cookie Dough, and the ability to purchase some rewards in-shop for participating girls who select Bonus Cookie Dough.

Paying for Cookies

  • Clover Go Login Request - Use this form to request a login for the Clover Go app, which allows you to take credit card payments at booth sales.

  • Clover Go User Guide - This guide covers how to use the Clover Go app to take credit card payments at booth sales.

  • Payment Adjustment Form - Fill out this form if you need to alter the amount withdrawn from the troop account due to extenuating circumstances.

  • Bad Check Form - Fill out this form if you receive a bad check/check with insufficient funds.

  • Girl Shortage Form - Fill out this form if you are having trouble collecting funds for cookies exchanged with a troop family.
